Safety and Security/Background

In order to secure the safety of the participants in our events, we must verify and consider carefully the background of our volunteers. A conviction will not necessarily disqualify an applicant. Failure to disclose will likely disqualify an applicant to volunteer. Some volunteer positions may require a background check. Please answer the following questions:

Pledge of Confidentiality/Photo Waiver

• This pledge of confidentiality is made and entered into between Global Down Syndrome Foundation (GLOBAL), Anna & John J. Sie Foundation (AJSF), Be Beautiful Be Yourself Fashion Show (BBBY) (collectively, “GLOBAL/BBBY/AJSF”), having its place of business at 3239 E. 2nd Ave, Denver, CO 80206 and having its events at various locations.
• This is to certify that I, a volunteer, understand that any information (written, verbal or other form) obtained during the performance of my duties must remain confidential. This includes all information about members, clients, celebrities, models, families, employees and other associate organizations, as well as any other information otherwise marked or known to be confidential.
• I understand that as a representative of GLOBAL, AJSF and BBBY, I am not allowed to take photographs of any attendees, celebrities or models during the BBBY event.
• I understand that as a representative of GLOBAL, AJSF and BBBY, I agree not to approach any attendees, celebrities or models for photos, autographs or any conversations other than those required to fulfill my responsibilities as a volunteer.
• I understand that I am not to consume any event food or beverage other than the food and beverage that will be provided for volunteers and located in the volunteer room.
• I understand that any unauthorized release or carelessness in the handling of this confidential information is considered a breach of the duty to maintain confidentiality.
• I further understand that any violation or threatened violation of this agreement will cause irreparable injury to GLOBAL/BBBY/AJSF, entitling GLOBAL/BBBY/AJSF to obtain injunctive relief in addition to all legal remedies.
• I agree and understand that this event will be photographed and videotaped. By entering the event, I grant Global Down Syndrome Foundation permission to use the likeness of all attendees for any future use.

Confirmation

• Have you responded to the above questions regarding any convictions (misdemeanor, petty offense, misdemeanor traffic conviction, felony or any other crime) honestly and accurately?
• Failure to disclose convictions may result in our rescinding your contingent offer to volunteer. Please be advised that we run a comprehensive national conviction search that often exceeds seven years.
• In consideration of my application for volunteer service, I authorize GLOBAL/BBBY/AJSF and all associated entities to conduct such an investigation and release all before mentioned companies from any liability or responsibility for this investigation, which may include, but is not limited to, the performance of medical examinations, drug screening, reference verification, driving history, military service and criminal background check which may be in the files of any state or local criminal justice agency. I understand that any information requested is for the sole purpose of gathering information accurately for use in the above-mentioned background check.
