GLOBAL & Denver Health Adult Down Syndrome Clinic – Vaccine Interest Form

Global Down Syndrome Foundation is excited to partner with Denver Health Foundation and our joint Adult Down Syndrome Clinic to offer COVID-19 vaccinations for individuals with Down syndrome and their family members ages 12 and older in Colorado.

If you are a person with Down syndrome age 12 or older who wants to receive the vaccine or if you are the family member (sibling, parent, grandparent) of a person with Down syndrome and you are 12 or older, please follow these steps:

1) Fill out the Vaccine Interest Form below (please fill out one form per individual with Down syndrome).

2) GLOBAL will then provide the information to Denver Health so you can be prioritized in vaccination scheduling.

3) A representative from Denver Health will contact you directly to complete your registration and schedule your vaccination appointment.

Registration is required. There are no walk-up appointments. COVID-19 vaccines will be available at Denver Health for all individuals with Down syndrome ages 12 and older on Fridays, Saturdays and Sundays beginning on Friday, May 14th.

• Children with or without Down syndrome ages 12 to 17 will be scheduled at the Denver Health main campus (see map below).

• Adults with with or without Down syndrome ages 18 and up will be scheduled at the at Denver Health’s Pena Southwest Family Health Center and Urgent Care.

 

COVID-19 vaccines for children with or without Down syndrome under age 12 are not yet available, but you can continue to check with Denver Health and the GLOBAL COVID-19 & Down Syndrome Vaccine Tracker for updated information.
