Summary: The Transition Medicine Clinic at Baylor College of Medicine provides medical care and social support services to the growing population of adolescents/young adults with a chronic childhood illness or disability as they graduate from the pediatric to the adult healthcare system. The only clinic of its kind in Texas, and one of only a few in the nation, The Transition Medicine Clinic assists these patients and their caregivers with accessing adult medical care, navigating the adult healthcare system, and maintaining or identifying alternative social services that are so critical for optimal health. We also serve as a unique resource to other healthcare professionals and the surrounding community, and as advocates for this patient population regarding local, state and federal healthcare initiatives.

Summary: This is a consultative clinic that provides a multidisciplinary approach to the care of children with Down syndrome from birth to 18 years. It is staffed by a pediatric geneticist from the University of Texas Southwestern Medical Center as well as specialty pediatric staff from Children’s Medical Center of Dallas. Care is provided according to the Medical Preventive Checklist for children with Down syndrome that has been endorsed by the American Academy of Pediatrics.
